St. Joseph’s Prep is dedicated to meeting the health and safety needs of students. Our certified school nurse is assigned to the Prep on Tuesdays, Thursdays, Fridays and every other Wednesday. Responsibilities include implementation of state mandated programs including immunization surveillance and student health screenings as well as students immediate health care needs delivering first aid and supportive nursing care.
All students must provide a completed Immunization Form with the dates of the Philadelphia Department of Health required immunizations.
All 9th grade students and transfer students are required to have a completed examination by your doctor or nurse practitioner. Physical examinations will be accepted if completed in the previous 12 months. PIAA sports physical form will also meet this requirement however this must be completed after June 1 of the current school year to meet the PIAA requirement.
Medication in School
Medication to be given on a regular or “as needed basis” requires a written authorization from your child’s health care provider every school year. This includes Epi-pens, insulin and inhalers, as well as oral medications. Have your child’s health care provider complete the Request for Administration of Medication Form.
Medication that needs to be given for treatment of an acute nature, such as oral antibiotics, requires a note from your health care provider stating the time of day and length of treatment. All medications must be sent to school in the original pharmacy labeled container.
Health Update Form is to be completed by all 9th grade and transfer students.
Student Health Status Form is to be completed for student with a history of a health problem yearly.
The deadline for all forms is July 31.
Pennsylvania state law mandates specific health screenings during the academic year. All grades will have growth and vision screenings and 9th grade will have hearing screenings as well. Parents of students who do not pass the health screenings will be notified by mail with a referral to the appropriate health care provider.
Nurse's Office Location & Hours
The Nurse's Office is located one the first floor between the Dining Hall and the Fieldhouse (behind the Dean's Office).
